Case Management

Case Management Eligibility Requirements

  • To qualify for Case Management services, individuals must:

  • Be 18 years of age or older.

  • Have a documented HIV diagnosis.

  • Be a South Carolina resident living in one of the seven coastal counties served by our program:

    • Charleston

    • Berkeley

    • Dorchester

    • Colleton

    • Georgetown

    • Horry

    • Williamsburg

  • Provide proof of income demonstrating eligibility at or below 550% of the Federal Poverty Level (FPL) for South Carolina.

Case Management Services

Eligible clients may access a variety of supportive services, including:

  • Individual Case Management

  • South Carolina AIDS Drug Assistance Program (ADAP) Assistance

  • Chaplain/Pastoral Care

  • Housing Assistance

  • Health Insurance Enrollment Assistance

  • Legal Aid Referrals

  • Life Skills Education

  • Medication Assistance

  • Peer Navigation Services

  • Smoking Cessation Support

  • Research Opportunities

  • Transportation Assistance

  • Translation and Interpretation Services
