Tiffany Green

Case Management Assistant

Tiffany Green serves as a Care Management Assistant at the HSC, supporting the care management team with patient needs, care coordination, documentation, discharge planning, community resources and Ryan White program activities. Through her role, she helps ensure patients have access to the support and resources they need throughout their healthcare journey.

Tiffany brings 11 years of experience working with 340B pharmacy programs and has completed 340B University training. She is currently a third-year student at South Carolina State University pursuing a bachelor’s degree in Sociology with a concentration in Medical Sociology. At the HSC, she continues to expand her knowledge of health services, care management and social work.

A Charleston-area native, Tiffany joined the HSC in August 2026. She was drawn to the organization by the opportunity to make a meaningful difference in people’s lives while bringing her previous healthcare experience into a new area of patient care. She especially enjoys helping patients find solutions, connecting them with resources and knowing that even small actions can make someone’s healthcare experience a little easier.

Compassion, respect, accountability and teamwork are central to Tiffany’s approach. She believes everyone has a story and strives to make each person she encounters feel valued, respected and cared for.

Outside of work, Tiffany enjoys spending time with family and friends, shopping, laughing and, especially, traveling and experiencing new cultures.
